How do I get the file?
Add it to your cart and check out for A$0. You will get an email with your download link, and the link also shows on your order confirmation page. The file is a ZIP: open it to find the PDFs in A4 and US Letter. If nothing arrives, check your spam folder, then email hello@holdingboth.com.au and I will send it again. The summary page has typeable fields. Open it in a free PDF reader such as Adobe Acrobat Reader to type into it, or print it and write by hand.

Which size do I print?
Every file comes in both A4 and US Letter. Use A4 if you are in Australia, the UK, Europe, New Zealand or most other countries, and US Letter in the US and Canada. When you print, choose 100% or "actual size", not "fit to page".

Can I share it with my brother or sister?
Yes, share the filled-in page with family, carers or the ward. If you would like to pass the blank page to someone, please send them to this page so they can download their own copy.

Can I get a refund?
It is free, so there is nothing to refund. If the file won't open or looks wrong, email hello@holdingboth.com.au and I will send another.

Who is it for?
